All scholar-athletes must have an approved physical before they are allowed to start any athletic season. Sports physicals are valid for 365 days from the date of the exam. If a physical expires during a specific sport, the student-athlete can continue playing until that sport is over but must obtain a new physical before starting their next sport.

Health History Update Questionnaire - If you submitted a physical form to the Athletics Office more than 90 days before the first day of official NJSIAA practice, you need to submit a Health History Update Questionnaire signed by the student’s parent or guardian to the Athletics Office.

Starting next school year, there will be a new procedure for physical packets. They will not be turned in to the school; instead, they should be kept by the healthcare provider who completes the physical exam.

The last page “Preparticipation Physical Evaluation Medical Eligibility Form” is the ONLY form that should be uploaded on the registration site or turned in to the school.

The physical exam must be completed by a healthcare provider who is a licensed physician, advanced practice nurse, or physician assistant who has completed the Student-Athlete Cardiac Assessment Professional Development module. This new process is designed to streamline the submission of physicals and ensure all necessary forms are in order.
